our ICU team don't have the capacity to do this anymore so we filter out on a monthly basis the patients coded as sepsis and call any who looks like they wouldn't have any support for example anyone in a residential/nursing home, cancer services - they have community support so we don't call those - we cannot call everyone so have to be pretty selective.
the patient feedback is very good though, they feel that they are still being cared for and we often refer them to social prescribers, or uk sepsis trust website for further support/info

We dont have one specifically for sepsis, there is one for post ITU admission that the outreach team attend along with an intensivist.
